Ordering your medications through Express Scripts© Pharmacy home delivery is a smart way to save time and money. Get a 90-day supply of maintenance medications delivered safely on your schedule, to your door. And there’s no delivery charge for standard shipping.
Getting started is easy. Visit express-scripts.com/mhp, download the Express Scripts© mobile app, or call the number on the back of your member ID card. You can also ask your doctor to send your prescriptions to Express Scripts© Pharmacy.
Once you activate your Express Scripts© Pharmacy account, you can transfer prescriptions, order refills, track shipments, and even set up automatic refills so medications arrive at your door when you need them.

Prescription Drug Reimbursement Claim Form
Use this form if you’ve paid up front for a prescription drug that your insurance covers and you’d like to be reimbursed.
You may also request a prescription reimbursement for your prescription electronically. Follow the instructions below to submit your claim online.
- Login to your member account on MyPlan
- From the Home page click on Online Pharmacy dropdown
- On the Online Pharmacy page, click the green Prescription Reimbursement button
- The Express Scripts (ESI) website will open, click Benefits and select Forms from the drop-down tab
- On the Forms page scroll down to Request Reimbursement and click the blue Start a Claim button
- Follow the instructions on the Get Reimbursed page and click the blue Get Started button when you are ready to begin your online claim
