Member Self-Referral

The Health Plan has a variety of free interactive clinical programs conducted by professional staff to help you achieve your care goals. We welcome your involvement and look forward to working with you.

You may use the web form or phone numbers below to enroll in any program. Program contacts may include telephone, mail, email, tele-video (HIPAA compliant Zoom) or member portal use as you prefer. Phones are answered Monday through Friday from 8 am to 5 pm (EST). Web referrals will receive a response within the next business day. 

Health, Wellness and Prevention Program (Core Wellness)

The Health, Wellness and Prevention Program (Core Wellness) provides education, support and resources if you need help improving your nutrition, achieving a healthy weight, increasing your physical activity, managing stress, or identifying and coping with depression.

Tobacco Cessation Program

The Tobacco Cessation Program delivers tips and techniques to assist you in decreasing or stopping tobacco use. Coverage of cessation aids and incentives are available with some benefit plans.

Life Coach Program

The Life Coach Program provides education, resources and interactive content to help you build  life skills useful in gaining employment, building resumes, managing your personal finances, managing a family/parenting, or finding legal aid in your community.

Social Determinants of Health (SDOH) Program

The Social Determinants of Health (SDOH) Program allows you to work with a THP social worker to make the most of your benefits and find extra resources in your community to assist with things like food, housing, transportation and caregiver services.

Pregnancy Care Programs

Pregnancy Care Programs are available to both high and low risk members, as well as to members who may want to discuss family planning or birth control options. Interactive educational content for every stage of pregnancy and newborn care is provided.

Chronic Disease Management Programs

Chronic Disease Management Programs are available and aimed at helping to educate members and support them in following their doctor’s recommended treatment plans related to certain chronic conditions. Members learning to live with and manage diabetes, coronary artery disease, congestive heart failure, chronic obstructive pulmonary disease and asthma are a perfect fit for this program. “Core Content”, THP educational materials, are available to help you learn about these conditions.

Case Management Programs

Case Management Programs are here to bridge gaps and coordinate care between you and your providers related to any medical conditions, behavioral health conditions, or substance use disorders. Program goals are tailored to meet your unique needs.

Personal goals may be focused on increasing your understanding of your disease or condition to help you self-manage and best express your wishes to your provider, managing or controlling symptoms impacting your daily life, understanding your available benefits and medications, or by removing any barriers to care through assistance with community resources.
