Changes to your contract information
The Health Plan strives to provide the most updated information to our members. Participating providers are asked to update their service address and other contact information when changes occur.
- You can contact your practice management consultant for your area.
- You can use the form below to submit your changes.
- You can go to findadoc.healthplan.org, search for the provider name and click on the "Verify/Update Practice Info" button.
- For large groups, call 1.877.847.7901 or email email@example.com for a form to fax back to us.
