Mountain Health Trust/Mountain Health Choices Services

Mountain Health Trust/Mountain Health Choices is a fully insured managed care plan offered to Medicaid-eligible residents of West Virginia.

The plan requires a member to select a Primary Care Physician (PCP), obtain a referral for specialty physician services and to follow precertification guidelines for procedures, diagnostic testing, outpatient surgical procedures, and inpatient admissions. Members do not have out-of-network benefits unless prior authorized by the Plan.

Pharmacy Services

The information provided on this website in the Pharmacy section is provided for Members with prescription coverage only. If you are uncertain about your coverage, please contact our Pharmacy Services Department at 1.800.624.6961, ext. 7914.

As a Health Plan member, you may obtain your prescription at any participating Health Plan network pharmacy. For the location of a participating pharmacy call Medco Health Solutions at 1.800.988.2262 or visit www.medcohealth.com.

In situations of emergency need for a prescription outside The Health Plan service area, please contact Medco Health for the location of a participating pharmacy in that area.

Members may request an exception for a drug that is non-formulary. The exception process allows The Health Plan to review member requests for non-covered pharmaceuticals.
