If you or a member of your family have a mental health or substance abuse crisis -- an episode that you feel threatens the life or health of anyone - you MUST get help immediately.
You should call 911 or go to the nearest emergency room or call your doctor, therapist or social worker for help.
If you are admitted to the hospital, after seeking emergency help, the hospital will contact The Health Plan. You do not need to worry about notifying us.
If you have a problem and need to speak to someone at The Health Plan, call the phone number listed on your ID card. This number, 1.877.221.9295, is answered 24 hours a day.
