What is an EMI? An EMI or Emergency Medical Information is a form that provides valuable information in a quick glance to medical personnel that need to take care of you. This would include emergency squad and emergency room staff, as well as any doctor taking care of you. The form includes your current and past medical history, allergies, medical devices, medications, and your list of doctors. It also contains important information about your daily physical health, such as notification that you are blind, hard-of-hearing, deaf, etc. and the primary language that you speak. Advance Directives and emergency contact names and numbers are included. Your doctor, nurse or a family member can help you fill out and keep current this valuable document if you are uncertain about what information goes where.
The Health Plan’s EMI is available in Member Files & Forms as a PDF download or you may call The Health Plan at 1.800.624.6961, Ext. 7659 or 740.695.7659 for a copy of the EMI form.
You may also choose to obtain a decal that can be placed on your front door to notify emergency personnel that EMI is available in your home. Free decals are available at www.vialoflife.com, or you can call the number above and we will send you an order form to use.
