Diabetes Program
This program is designed to modify risk factors associated with diabetes as well as slow the progression of microvascular and macrovascular complications by promoting treatment plan compliance through education, counseling, and support. Members with diabetes require long-term, continual health care to maintain appropriate glycemic control and to decrease the risk of long term complications such as neuropathy, nephropathy and blindness.
